A one-story-over-garage hipped-roof dwelling in rustic channel siding, which the survey reads as part of a row with 1086 and 1092 Innes Avenue. The survey's record for 1064 Innes Avenue attributes five dwellings on the north side of the 1000 block — 1064, 1068, 1080, 1086 and 1092 — to the speculative builder George M. Moore, who bought the lots in March 1938 shortly after the block was graded from Hawes Street to Ingalls Street.
