The Board of Supervisors got the legislature's authority to build and run an alms house and hospital, and set aside eighty acres of City land on the west side of Twin Peaks for it.

1867
The Alms House opened. The first building was a four-storey Second Empire block with three wings off a central core, built for 400 to 500 patients and overlooking the Spring Valley Water Company's Lake Honda; a separate 40-bed infirmary stood beside it.

The 340-bed infirmary on the Laguna Honda campus was built over 1907 and 1908, among the first concrete buildings put up after the fire.

Clarendon Hall was built on the Laguna Honda campus to the design of the San Francisco Bureau of Architecture and Newton Tharp — the oldest surviving building there, Classical Revival in style, and one of the first hospital buildings put up after the earthquake.

The campus garage was built at Laguna Honda; an addition was later put on its east end.

The main administrative block and Wings A to G — the oldest part of the Main Hospital Building at Laguna Honda, in the Spanish Revival manner, with five-storey wings running off a central corridor — were built to the design of John Reid, Jr., the City Architect.

Service buildings went up as the Laguna Honda campus was expanded: the bridge building spanning the valley between the Main Hospital Building and Clarendon Hall, the greenhouse on the north side of the Main Hospital Building, and the laundry.

Wings K and L were added to the Main Hospital Building at Laguna Honda, extending it eastward up the hill.

A four-storey addition of 196 beds was built at Laguna Honda hospital, with Splivoch & Splivoch as the builders.

Wings M and N of the Main Hospital Building at Laguna Honda, now called Wing O, were added.

1935–1941
Works Progress Administration workers reconditioned the wards of the Laguna Honda Home for the Aged and Infirm to improve capacity and comfort. New Deal artists worked on the campus too: Glen Wessels painted five murals in the main lobby — The Professions and Earth, Air, Water and Fire — and David Lindstrom carved a cast-stone Florence Nightingale for the main entrance.

The Juvenile Court and Youth Guidance Center, designed by architect William Gladstone Merchant, who trained under Bernard Maybeck and John Galen Howard.

The shop building was built on the Laguna Honda campus.

July 15, 1971
A Standard Oil gasoline service station occupied the city-owned site at Woodside Avenue and Portola Drive, just east of the Youth Guidance Center. The City Planning Commission found a proposed fifteen-year lease of the site for a gasoline service station in conformity with the Master Plan, subject to its review of remodeling or rebuilding plans.

On the Laguna Honda Hospital grounds, the hospital's power plant and laundry facilities stood at the edge of the open-space district, near a hollow at the northeast corner of the site holding the Twin Peaks Tunnel's existing ventilation shaft. The City Planning Commission unanimously found the transfer of jurisdiction of a 50-by-100-foot plot on the Laguna Honda Hospital grounds, for a Municipal Railway transformer substation, in conformity with the Master Plan, provided the building was painted dull green and screened with dense landscaping.
